IBHP is currently hiring practice coaches to work as part of a team with the nation’s largest Medicaid health plan on a federally funded effort to provide the technical assistance needed to transform care for individuals served in the safety net.
Background and Opportunity
L.A. Care, the nation’s largest Medicaid health plan, has received a federal award of up to $15.8 million over four years to implement a Practice Transformation Network in Los Angeles (LAPTN). LAPTN aims to improve health outcomes and experience of care for individuals served by the safety net system
L.A. Care has retained IBHP to provide practice coaches to work with network partner organizations (primarily DMH) over the next three years.
The goal for this project is to develop a practice transformation and service delivery model that will achieve:
- improved diabetes screening and control,
- improved screening and treatment planning for depression,
- improved nephropathy monitoring,
- improved follow up and care transitions after hospitalization for mental illness,
- reduced unnecessary lab testing,
- reduced hospital admissions, and
- lower costs.
These outcomes are consistent with the Triple Aim that is being promoted by broader Federal and State health reform efforts – Improved Population Health, Improved Patient Experience, and Reduced Costs.
Technical assistance will be tailored to meet practice needs and may include provider outreach and education, practice workflow redesign, change management and strategic planning. Technical assistance will be provided at practice sites and remotely guided by transformation work plans developed by L.A. Care and the Network Partners. Coaches will participate in coach workgroups and learning sessions to share learnings and gain knowledge to better provide technical assistance
- Bachelor’s degree in health care or related field. Experience in clinical management, process improvement, etc., may be substituted for education.
- 5+ years’ hands-on experience in the health care industry, including behavioral health and human services settings
- Demonstrated strong project management, communication and relationship management skills utilized in a fluid program environment.
- Must have implemented a Plan-Do-Study-Act (“PDSA”) or similar quality improvement methodologies using a data driven methodology.
- A successful history of implementing workflow redesign in practices using EHR systems.
- Proven ability to work in a team environment and manage issue resolution effectively.
- Ability and willingness to travel throughout Los Angeles County, as needed
Coaches will provide onsite and remote services for network partners in Los Angeles County as contracted consultants for IBHP.
- Execute work plan for individual practices and provide feedback to help update relevant areas of work plan using LAPTN project management tool.
- Provide technical assistance to individual Practices in transforming their practices for treating patients.
- Tailored to meet practice needs and may include supporting provider outreach and education, practice readiness, practice facilitation, health information technology (“HIT”), optimization, practice workflow redesign, change management and strategic planning.
- May be provided on-site and remotely using a set of intervention work plans and interventions as defined by L.A. Care and the Network Partners to help Practices improve quality measures and transform their practices.
- Participate in coach workgroups and other learning sessions to share learnings and gain knowledge to better provide facilitation/teaching and technical assistance.
- Track defined clinical quality measures in the Practices’ electronic health records (“EHR”) system, and send them to the LAPTN clinical data repository department, as required.
- As required, conduct ongoing practice assessments using PAT.
