Clinical and Community Collaboration to Achieve Collective Impact
In the wake of the Affordable Care Act, numerous federal and California health care initiatives are underway to transform usual care practices to improve the health and well-being of the population, with a particular focus on individuals with complex health and psychosocial conditions. Within these initiatives, there is an expectation for cross-sector collaboration, coordination, and data sharing to better address the social determinants that profoundly impact health outcomes and costs of care.
The Summit’s goal is to learn from the experiences of regional, cross-sector partners who are expanding and redefining their roles, and working to collectively build and implement coordinated and accountable systems of care. Leaders of health and behavioral health plans, primary care, mental health and substance use disorder systems, hospitals, homeless service providers, and others will discuss and explore innovative strategies for patient engagement and activation, service delivery, and payment and financing reform.
